On behalf of the Board of Directors of the Council of Church-Based Health Programs, Inc. (CCBHP) and all of our local Health Advisory Councils (HAC), we appreciate your interest in joining the ranks of those who have decided to make health, education and community development a priority.
The Council of Church-Based Health Programs, Inc. is a statewide non-profit interfaith consortium of churches and church leaders, concerned with the health and human service needs of the communities in the State of Florida. If your local church would like to become a part of this council, please fill out the application and attached forms below. Membership is free and there is no obligation to join. However, your commitment and dedication are sought in order to make this organization realize its purposes and mission. Various fund raisers and other community focused health and educational initiatives will be required of all participating churches. If you desire additional information, please contact:
